関節リウマチは，膝関節でベーカー囊腫を合併しやすく，まれに破裂することもある．特に両側性の破裂の頻度は低い．ベーカー囊腫破裂は，臨床上，下肢深部静脈血栓症に似た症状を認め，偽性血栓性静脈炎と称される．症例は，70代女性．主訴は，両側下肢腫脹と疼痛．現病歴は，当院受診1週間前から突然の両側下肢腫脹と疼痛が出現し増悪したため，近医の整形外科を受診し下肢深部静脈血栓症が疑われ，当院を紹介受診した．下肢静脈超音波検査では，両側ともに血栓像は認めなかった．しかし，両側膝窩部の腓腹筋内側頭と半膜様筋腱間から発生した内部エコー不均一な低エコー像を認め，下腿へと連続し広がっていたことから両側ベーカー囊腫破裂を疑った．造影CT検査でも同様の所見であり，両側ベーカー囊腫破裂と診断された．消炎鎮痛剤投与と弾性ストッキング着用による保存的療法を行い症状の改善を認めた．関節リウマチに合併した両側ベーカー囊腫破裂はまれであり，臨床症状のみでは下肢深部静脈血栓症との鑑別は困難であることが多く，超音波検査が有用であった．
Bilateral ruptured Baker’s cysts are a rare cause of acute lower extremity swelling accompanied by pain. The differential diagnoses for lower leg swelling with pain include various musculoskeletal and vascular pathologies such as deep venous thrombosis. Ultrasound imaging plays a crucial role in the swift and accurate detection of such diseases. Herein, we report a case of bilateral ruptured Baker’s cysts in a patient with rheumatoid arthritis presenting with bilateral lower extremity swelling and pain. The diagnosis was confirmed via ultrasound, and successful management was achieved with medication.
